    By signing this agreement, I acknowledge that participation in Brazilian Jiu-Jitsu,
    grappling, self-defense training, conditioning drills, sparring, and related physical
    activities involves inherent risks.
 

1. Assumption of Risk

    I understand that training may result in injury, including but not limited to bruises,
    sprains, strains, dislocations, broken bones, concussions, serious bodily injury, or death.
    I voluntarily choose to participate and fully accept all risks, known and unknown.
 

2. Release of Liability

    I hereby release, waive, and discharge Pythagoras Jiu-Jitsu North, its
    owners, instructors, coaches, staff, volunteers, affiliates, landlords, and fellow
    participants from any and all claims or causes of action arising from participation in
    training, classes, private lessons, open mats, competitions, events, or use of the
    facility, to the fullest extent permitted by Florida law.
 

3. Medical Responsibility

    I certify that I am physically and medically able to participate. I understand that the
    school does not provide medical insurance. I agree to assume full responsibility for any
    medical expenses incurred and authorize emergency medical treatment if necessary.
 

4. Rules, Conduct & Safety

    I agree to follow all gym rules and instructor directions, train in a controlled and
    respectful manner, maintain proper hygiene and attire, and immediately report injuries or
    unsafe conditions. Failure to follow rules may result in removal from class or termination
    of membership without refund.
 

5. Photography & Media Release

    I grant permission for Pythagoras Jiu-Jitsu North to use photographs or
    videos of me (or my child) taken during training or events for promotional, educational, or
    marketing purposes, without compensation.
 

6. Minors

    If the participant is under 18 years of age, the parent or legal guardian agrees to all
    terms of this waiver on behalf of the minor and accepts full responsibility for the
    minor’s participation.
 

7. Governing Law

    This agreement shall be governed by and interpreted under the laws of the State of Florida.
 

    Participant and parent/guardian (if applicable) acknowledge that electronic signature
    constitutes a legally binding agreement.
